General Info Required to Fill Passport Online Application

The following information should be kept ready to be filled online for applying for Passport Online:
1. Date of Birth of the Applicant
- For applicants born on or after 26/01/1989, only Birth Certificate issued by the Municipal Committee or any office authorized to issue Birth and Death certificate issued by Registrar of Births and Deaths is acceptable. Birth Certificate should include name of the child, name of father and mother, place of birth, sex, registration number and date of Registration. However in case the the Birth Certificate does not include the name of the child, a declaration on plain paper signed by parents, is required to be submitted specifying the name of the child.
- School Leaving Certificate/Certificate of Recognized Boards from the school last attended by the applicant or any other recognized educational institution can be accepted for applicants who have passed Matriculation or above.
- Affidavit sworn before a Magistrate/Notary stating date/place of birth as per specimen in Prescribed Affidavits by illiterate or semi-illiterate applicants (Less than 5th class)
2. Place of Birth with Village, PO, Tehsil, District and Pin-Code.
3. Martial Status
4. Employment if Any (For Government Employees NOC is required as per Prescribed Annexures.
5. Educational Qualifications.
6. Visible Distinguish Mark on the Body
7. Aadhaar Number of Applicant
8. Father’s and Mother’s Name
9. Residing at Current Address Since ______________ along with name of Police Station
10.Complete Address of the applicant along with Pin Code
11.Mobile Number of the Applicant
12.Email ID of the Applicant
13.Name, Addresses and Mobile Numbers of Two References
14.Emergency Contact – Mobile Number, Name and Address
15.Proof of Present Address – which can be Water/Telephone/Electricity Bill, Photo Pass Book of running Bank Account, IT Assessment Order, Voter/EPIC Card, Gas Connection Bill, Spouse’s Passport Copy, Employer’s ID Card of reputed and widely known companies, Parent’s passport copy in case of minors (first and last page)
16.Aaadhar Card issued by UIDAI
17.Standard Affidavit as per Prescribed Affidavits
